same here.

having said that i really struggle with heel-toe when hill-climbing and sprinting. its not second nature yet so i still have to think about my foot position. i dont really get time for that but at the end of the trackday yesterday it was much more natural. footwear does make all the difference although for me with kart boots i had to make a new throttle pedal cover so i could actually reach the throttle!

I lift my leg for the clutch, I "heel-toe" with the ball of my foot on both pedals and just pivot it the left side of my foot onto the throttle when I brake.

But I don't drive on track where I imagine it's a bit more hectic to do it all properly...

From the images your description brings up in my mind that must hurt. I presume you mean the right side of your foot!

You can get a modern version with more sole padding.

Thanks for all the interesting replies. Regarding rocking the foot rather than actually using toe and heal: this did cross my mind, but I just can't feel the pedals well enough to be sure that my foot is in the right place. I think I need to get some driving shoes. Those Pilotis look fabulous!
